Tribulus terrestris and steroidal saponins: bioactive compounds and hormonal mechanisms of action:
Tribulus terrestris contains multiple phytochemicals with documented biological activity: flavonoids (such as quercetin and kaempferol), alkaloids, tannins, resins and, above all, steroidal saponins (the class of compounds responsible for hormonal effects). Tribulus terrestris steroidal saponins are molecules with a molecular structure similar to endogenous steroids (testosterone, estrogens, progesterone), consisting of a steroid nucleus (sapogenin) linked to sugar chains. The predominant sapogenin of Tribulus is diosgenin, and the most studied bioactive saponin is protodioscin (3-O-[α-L-rhamnopyranosyl-(1→2)-β-D-glucopyranosyl] -22-O-[α-L-rhamnopyranosyl] diosgenin), which accounts for 40 to 45% of total saponins. Protodioscin is converted in the body to DHEA (dehydroepiandrosterone) by intestinal and liver enzymes, and DHEA, in turn, is a precursor of testosterone and estrogens in the periphery. This "plant pro-hormone" mechanism allows Tribulus to support the endogenous synthesis of androgens without introducing exogenous androgens, unlike synthetic anabolic steroids.
Testosterone stimulation via luteinizing hormone (LH): the documented central mechanism:
The most documented mechanism of action of Tribulus terrestris on the gonadal axis is the stimulation of luteinizing hormone (LH) secretion by the pituitary gland. Steroidal saponins, by interacting with pituitary receptors (possibly via activation of estrogen receptors in the pituitary by diosgenin, which can act as a partial agonist of estrogen α receptors), stimulate the pulsatile secretion of LH. LH (produced by pituitary gonadotrophic cells) is the main endocrine signal that stimulates Leydig cells in the testes to produce testosterone. By increasing LH, Tribulus secondarily increases testicular testosterone synthesis through a physiological endocrine mechanism (unlike exogenous androgen administration, which suppresses LH and the HPA axis via negative feedback). This mechanism is particularly relevant for men with subnormal testosterone levels due to functional hypogonadism (chronic stress, overtraining, sleep deficiency, nutritional deficiency) where testicular synthesis capacity is intact but pituitary stimulation is reduced.
Clinical evidence on Tribulus terrestris: documented effects and limitations:
The clinical evidence base for Tribulus terrestris is more heterogeneous than that of Ashwagandha KSM-66® or other newer adaptogens. Several clinical trials document positive effects on libido, erectile function, and sexual health markers, especially in men with mild to moderate erectile dysfunction. The study by Neychev & Mitev (2005) documented that 750 mg/day of Tribulus terrestris extract for 5 days increased LH levels by 72% and free testosterone levels by 41% in healthy athletes. The study by Milasius et al. (2009) documented improvements in sports performance parameters in basketball players after 20 days of Tribulus supplementation. However, other clinical trials (especially those conducted in men with normal testosterone levels) did not document significant increases in testosterone, suggesting that the effect may be more pronounced in men with subnormal baseline levels. Dose and standardization (% saponins) are critical factors: most studies that found no effect used extracts with saponin content below 40%, while positive studies used extracts with 45 to 90% saponins. Regardless of the controversy about the effects on serum testosterone, Tribulus terrestris has a more consistent evidence base on the effects on libido and sexual function in both sexes. Proposed mechanisms include: central action on androgen receptors in the hypothalamus (which regulate sexual behavior and libido); increased nitric oxide (NO) in penile arteries via activation of the endothelial nitric oxide synthase (eNOS) system by Tribulus flavonoids, improving penile blood flow (a mechanism similar to sildenafil, but with lower potency); and effects on dopaminergic neurotransmitters that regulate sexual desire. Tribulus saponins have moderate adaptogenic properties that contribute to maintaining energy and resistance to fatigue, especially in contexts of physical stress and intense training. The increase in testosterone (when it occurs) improves muscle protein synthesis, maximum strength, post-workout recovery, and motivation for training (testosterone has effects on central dopamine that translate into greater willingness and positive aggression in training). In addition to hormonal effects, Tribulus flavonoids have antioxidant activity that can reduce oxidative stress from intense exercise.
Prostatic health and urinary system:
Tribulus terrestris saponins have documented anti-inflammatory and antispasmodic effects on the smooth muscle of the urinary tract, with evidence of reducing symptoms of mild benign prostatic hyperplasia (BPH) in some studies, namely improved urinary flow and reduced urinary urgency. These effects are distinct from effects on testosterone and may benefit older men with mild urinary symptoms.
Uses
Recommended dose: Take 1 to 2 capsules/day with water, preferably at night before bed (to take advantage of the peak nocturnal secretion of LH and testosterone, which occurs during deep sleep and is potentiated by Tribulus). To start, begin with 1 capsule/day during the first week and progress to 2 capsules/day if well tolerated. Take a 2-week break after every 6 to 9 weeks of continuous supplementation to avoid tachyphylaxis (reduced response due to continuous use) and maintain receptor sensitivity.
