Ashwagandha (Withania somnifera L., Solanaceae family) is an evergreen shrub native to India, North Africa and the Mediterranean region, used in Ayurvedic medicine for over 3000 years as a "rasayana" (tonic and rejuvenator). It is classified as an adaptogen: a compound that increases the body's non-specific resistance to multiple types of stress (physical, chemical, biological), without producing excessive stimulation and without creating dependence. Its most relevant active compounds are withanolides (steroidal lactones with a similar structure to steroid hormones) and withanamides (alkaloids), mainly present in the root. KSM-66® differs from conventional Ashwagandha extracts in three technical characteristics: it is extracted exclusively from the root (where withanolides are more concentrated and have a better phytochemical profile), it uses a patented milk-based extraction process (in a process inspired by traditional Ayurvedic medicine that recommended boiling the root in milk) that preserves the full spectrum fat-soluble compounds, and it is standardized to 5% withanolides with the highest batch consistency among all ashwagandha extracts on the market.
Cortisol reduction and HPA axis modulation: the central mechanism of adaptogenic effect:
The main mechanism of action of Ashwagandha KSM-66® is the modulation of the hypothalamic-pituitary-adrenal (HPA) axis, the central neuroendocrine axis of the stress response. In situations of chronic stress, persistent activation of the HPA axis (via hypothalamic CRH → pituitary ACTH → adrenal cortisol) results in chronic hypercortisolism, with multiple negative effects: immunosuppression, muscle catabolism (cortisol promotes muscle proteolysis via glucocorticoids), insulin resistance, cognitive and sleep deterioration, and suppression of gonadal activity (elevated cortisol suppresses GnRH secretion and, consequently, testosterone). Ashwagandha's withanolides act via several mechanisms on the HPA axis: they modulate glucocorticoid receptors (GR) in target tissues, reducing sensitivity to cortisol fluctuations; they inhibit the enzyme 11β-HSD1 (which converts inactive cortisone into active cortisol in peripheral tissues); and they reduce the expression of pro-inflammatory genes activated by cortisol (via NF-κB). The randomized clinical trial by Chandrasekhar et al. (2012), published in the Indian Journal of Psychological Medicine with 64 adults with chronic stress, documented that 300 mg of KSM-66® twice daily for 60 days reduced serum cortisol levels by 27.9% compared to placebo.
Reduction of stress and anxiety: the most documented and consistent effect in clinical trials:
Ashwagandha's withanolides have documented anxiolytic effects via multiple mechanisms: GABAergic modulation (withanolides have an affinity for GABA-A receptors, with anxiolytic effects similar to benzodiazepines but without dependence or excessive sedation), cortisol reduction (which secondarily reduces amygdala hyperexcitability associated with chronic stress), and anti-inflammatory effects in the CNS (neuroinflammation contributes to anxiety and depression). The clinical trial by Langade et al. Improved sleep quality: the effect secondary to cortisol reduction and GABAergic effect:
Ashwagandha improves sleep quality through two main mechanisms. First, the reduction of nocturnal cortisol: cortisol has a circadian rhythm with a morning peak and a nocturnal nadir, and chronic hypercortisolism can elevate nocturnal cortisol levels, impairing sleep latency and sleep architecture (elevated cortisol reduces slow-wave sleep). By reducing basal cortisol levels, Ashwagandha promotes the normal transition to deep sleep. Second, the GABAergic effect of withanolides reduces nocturnal neuronal excitability and cognitive rumination that often prevents falling asleep. In addition to these mechanisms, recent studies have identified that triethylene glycol (a component different from withanolides present in the leaves and in smaller amounts in the root) has documented sleep-inducing properties in animal models, contributing to the plant's overall effect on rest.
Increased muscle strength and athletic performance: the ergogenic effect of Ashwagandha:
In addition to its effects on stress and sleep, Ashwagandha KSM-66® has multiple clinical trials documenting relevant ergogenic effects for athletes. The randomized clinical trial by Wankhede et al. (2015), published in the Journal of the International Society of Sports Nutrition with 57 young males, documented that 300 mg of KSM-66® twice daily for 8 weeks of resistance training resulted in greater gains in maximal strength (bench press and leg extension), greater gain in muscle mass, greater reduction in body fat percentage and less muscle damage (lower elevation of serum CK after training) compared to placebo. The mechanism involves multiple factors: cortisol reduction removes the catabolic signal that limits muscle protein synthesis; the secondary increase in testosterone (which occurs when cortisol decreases, given that cortisol suppresses testicular testosterone production via the HPA-HPG axis); and the anti-inflammatory effects of withanolides that reduce exercise-induced muscle damage and accelerate recovery.
Support for male hormonal health and fertility:
Ashwagandha's reduction of cortisol removes the suppression that hypercortisolism exerts on the hypothalamic-pituitary-gonadal (HPG) axis, resulting in increased levels of LH (luteinizing hormone), FSH, and testosterone in men with chronic stress. The clinical trial by Mahdi et al. (2009), with 75 men with oligospermia (low sperm count), documented that 5 g/day of Ashwagandha root powder for 3 months significantly increased sperm concentration (+167%), sperm motility (+57%), and testosterone levels (+40%) compared to placebo. KSM-66® specifically has been tested in clinical trials for male reproductive health, with similar results at lower doses (600 mg/day).
Cognitive function and memory: documented nootropic support:

Uses
Recommended dose: Take 1 capsule/day (500 mg of KSM-66®, 25 mg of withanolides), preferably at night 30 to 60 minutes before bedtime, to take advantage of the sleep improvement and nocturnal cortisol reduction effect. For athletic performance goals or when the maximum effect is desired (equivalent to the doses used in clinical trials of 300 mg twice daily = 600 mg/day), 1 capsule can be taken in the morning with a meal + 1 capsule at night, not exceeding 2 capsules/day. The effects of Ashwagandha are cumulative and depend on regular medium-term use: clinical trials document significant effects from 4 to 8 weeks of daily use.
