Boswellia serrata (Shallaki, or Indian frankincense) is the resin obtained from the bark of Boswellia serrata, a tree native to the Indian subcontinent. Its active compounds are boswellic acids (especially AKBA — acetyl-11-keto-beta-boswellic acid — and KBA — keto-beta-boswellic acid), which have a unique anti-inflammatory mechanism of action distinct from pharmacological NSAIDs: they specifically inhibit 5-lipoxygenase (5-LOX), the enzyme that catalyzes the synthesis of leukotrienes (the inflammatory mediators responsible for chronic joint inflammation and neutrophil migration to the joints). Unlike NSAIDs (which inhibit COX-1 and COX-2 and carry a risk of gastrointestinal injury with prolonged use), Boswellia does not inhibit protective gastric mucosal prostaglandins — which is why it has a superior gastrointestinal safety profile for long-term use. Boswellia also improves blood supply to the joints (by restoring the integrity of vessels destroyed by arthritic vascular spasms) and reduces the degradation of glycosaminoglycans (GAGs) in joint cartilage — which is the main cause of cartilage destruction in osteoarthritis and rheumatoid arthritis. Guggulu (Commiphora wightii, Indian Bdellium) is the resin of Commiphora wightii — the Indian equivalent of Commiphora myrrha in traditional Arabic medicine. Its active compounds are guggulsterones (guggulsterone E and Z, plant steroids) with multiple pharmacological properties relevant to the joint. Guggulu is an inhibitor of nitric oxide production in joint chondrocytes and synoviocytes — excessive nitric oxide, produced during joint inflammation, causes oxidative damage to cartilage proteins (oxidation of collagen and proteoglycans) and stimulates chondrocyte apoptosis (cell death that accelerates cartilage loss). Usage
